{"id":19789,"date":"2020-02-05T18:48:42","date_gmt":"2020-02-05T18:48:42","guid":{"rendered":"https:\/\/imatsolutions.com\/?p=19789"},"modified":"2025-10-07T21:09:32","modified_gmt":"2025-10-07T21:09:32","slug":"upmc-health-plans-new-social-impact-initiative","status":"publish","type":"post","link":"https:\/\/imatsolutions.com\/index.php\/2020\/02\/upmc-health-plans-new-social-impact-initiative\/","title":{"rendered":"SDOH Leader: UPMC Health Plan\u2019s New Social Impact Initiative"},"content":{"rendered":"<p>As we <a href=\"https:\/\/imatsolutions.com\/index.php\/2020\/01\/2020-already-becoming-year-of-social-determinants-of-health-for-payers\/\">highlighted<\/a> last month, 2020 is already shaping up to be the year of Social Determinants of Health (SDOH) for health payers.<\/p>\n<p>In this blog post, we discuss how University of Pittsburgh Medical Center (UPMC) Health Plan\u2019s Social Impact effort will translate the work of its unique payer-provider laboratory into evidence-based practice and policy change for improving health care quality and efficiency.<\/p>\n<p>\u201cThe goal of UPMC Social Impact is to help meet the health, socioeconomic, and environmental needs of UPMC Health Plan members and the communities we serve,\u201d said John Lovelace, president of government programs at UPMC Health Plan, in <a href=\"https:\/\/www.globenewswire.com\/news-release\/2019\/12\/19\/1963104\/0\/en\/UPMC-Health-Plan-Announces-New-Social-Impact-Initiative-to-Help-Address-the-Social-Determinants-of-Health.html\" target=\"_blank\" rel=\"noopener noreferrer\">this press release<\/a>. \u201cThe initiative will create new social responsibility programs and expand upon our existing programs to positively impact the health and wellbeing of our members, employees, and the neighborhoods where we live and work.\u201d<\/p>\n<p><em>Health Payer Intelligence <\/em>recently <a href=\"https:\/\/healthpayerintelligence.com\/news\/how-upmc-coordinates-compounding-social-determinants-of-health\" target=\"_blank\" rel=\"noopener noreferrer\">published an article<\/a> that highlights how this program, along with other SDOH efforts at UPMC, are paving the way for other payers to best address population health challenges.<\/p>\n<p>For example, the plan\u2019s Cultivating Health for Success program focused on partnering with a local housing and urban development vendor to coordinate a team-based approach that integrates permanent housing and intensive case management for individuals experiencing homelessness.<\/p>\n<p>In the first five years of the program, UPMC <a href=\"http:\/\/modernmedicaid.org\/medicaid_solutions_upmc_for_you\/\" target=\"_blank\" rel=\"noopener noreferrer\">showed a savings<\/a> of more than $6,384 for each of the 51 members it was able to house.<\/p>\n<p>The article also discusses how health data exchange is a key part of this effort.
