Measures Used in Overall Approach
Number of single patient exports and number of bulk patient exports
Description of Measurement
Count of single patient requests, count of single patient CCDs generated and count of single patient CCDs downloaded. Count of bulk patient requests, count of bulk patient CCDs generated and count of bulk patient CCDs delivered.
Associated Certification Criteria
Modules: 170.315(b)(10)
Justification for Selected Measurement
The export of EHI associated with a patient is another way to share information with an external organization. Export is typically used when there is a need for a full patient record. This metric will provide information on the type of data exported for a single patient and the frequency of usage.
Care Setting
IMAT has received certification through NCQA as Validated Data Stream under their Data Aggregator Validation(DAV) program. IMAT will focus on care settings which have been validated under the DAV program. These currently include the Outpatient Care and Outpatient Lab care settings.
Expected Outcomes
It is expected that authorized users will be able to share EHI using the CCD on demand API. Error rates will be tracked and trended over time.
Number of patients covered in data reported to CMS and TJC
Description of Measurement
Count of patients that IMAT has received QRDA I records or other clinical data for that were represented in the generated QRDA III records.
Associated Certification Criteria
Modules: 170.315(c)(2),(c)(3)
Justification for Selected Measurement
Shows IMATs ability to generate QRDA III records for use in submissions to CMS and TJC.
Success of this product relies on the ability to generate QRDA III records that can be reported to CMS and TJC, so any amount greater than 0 will be deemed to be a success. From this total benchmarks will be established for future years to ensure that the product continues to grow and provide more data for CMS and TJC.
Percentage of QRDA III’s generated that were submitted to CMS and TJC
Description of Measurement
Companion measure used to track the percentage of QRDA III’s that were generated. This will be used to track quality issues that could affect reporting.
Associated Certification Criteria
Modules: 170.315(c)(2),(c)(3)
Justification for Selected Measurement
Tracks the quality of data generated

IMAT has received certification through NCQA as Validated Data Stream under their Data Aggregator Validation(DAV) program. IMAT will focus on care settings which have been validated under the DAV program this currently includes the Outpatient Care and Outpatient Lab care settings.
Expected Outcomes
IMAT targets 100% of all QRDA IIIs generated will pass a QRDA III structure validation tool, however the target will be adjusted in subsequent validations to reflect real world outcomes based on actual data.
Number of issues found requiring attention per eCQM measure
Description of Measurement
Companion measure used to track the stability of the product across each eCQM measure. The success of the product relies on the ability to produce useful data that can be submitted to CMS and TJC.
Associated Certification Criteria
Modules: 170.315(c)(2),(c)(3)
Justification for Selected Measurement
Tracks the quality of the product submitting data to CMS and TJC
Care Setting
IMAT has received certification through NCQA as Validated Data Stream under their Data Aggregator Validation(DAV) program. IMAT will focus on care settings which have been validated under the DAV program this currently includes the Outpatient Care and Outpatient Lab care settings.
Expected Outcomes
IMAT targets 0 issues per measure. New targets will be created on subsequent validations based on issues that have been found and their target dates for resolution.
